Certificate of Immunization

All new regularly admitted undergraduate and graduate students will be required to submit a Certificate of Immunization prior to attending classes.

Students who have previously attended a state public college/university are not exempt from this updated, revised immunization requirement.

The  requires that all students provide proof of immunity to measles, mumps and rubella. Effective January 2005, new students are also required to provide proof of immunization to varicella (chicken pox), tetanus, diphtheria and, if 18 years of age or less at the time of classes, hepatitis B. The purpose of this policy is to ensure that students are protected against communicable diseases that are preventable and to reduce the likelihood of an epidemic or threatened epidemic on a Georgia public college/university campus.

Although admission decisions will be made without the 91ÁÔÆæ Certificate of Immunization form students will need to provide the required proof of immunization in order to remain enrolled in first semester courses. We strongly encourage students submit proof of immunization at least a month prior to the start of their first semester at 91ÁÔÆæ in case an updated immunization is needed.
