Worker's Compensation
All employees of the University are covered under the provisions of the Workers’ Compensation Act. This act provides protection for employees in the event of injury or death while performing services for 91ÁÔÆæ. Employees who incur an on-the-job injury should immediately notify their supervisor so that the necessary paperwork may be completed in 5 days as required by law.
In the event of an emergency worker’s comp injury, where the employee requires immediate medical attention, you should proceed to the nearest emergency medical care facility or call 911 for assistance. Immediately following the emergency care, the supervisor must call DOAS at 1-877-656-7475 and report the injury. The employee or supervisor would then call AmeriSys Managed Care at 678-781-2848 or 1-800-900-1582 concerning follow up care. If additional care is needed, a Case Manager will assist the employee with selecting an authorized treating physician for follow up care.
In the event of a possible non-emergency worker’s comp injury, the following guidelines should be followed. Employee must report the injury to their supervisor immediately, but no later than 24 hours. The supervisor will continue to call DOAS at 1-877-656-7475 to report the claim and give the following information:
- Name, address, social security number, age, and sex of the injured employee
- Name of employing agency, address, and phone number
- Date, time, and description of accident (when, where, how, why)
- Part of body injured and type of injury (cut, scrape, burn, etc.)
- Hourly, weekly, or monthly wage
- Name and address of physician and/or hospital
- Has injured employee returned to work?
If no medical care is needed, complete the Incident Report Only form. Do not call DOAS to report. Supervisors should either hold this report or send to Risk Management. Do not call DOAS to report.
